I would think that it was totally staged. Dr's don't take that much time with one patient when they have cancer, much less checking for muscle tone or hernia.
However -- It is more than possible to experience it. Don't know where you live, but get you local Gay publication and go to the want ads. Go through the list of guys that give massages and call and talk to them. You can even offer to send the the address to that video so they know what you want. The guys I have had "massages" from are very willing to do most anything to please.
