www.koalaswim.com has the most ridiculously small suits I've seen.
There are a few with backs, though pretty much all are not really suitable for public.

Groovin' underwear. I got some from Thailand earlier this year. Some of their styles are very lycra. This page says they are Japanese but I'm not sure that's right. Try the sportswear shops in the sporting goods district in Kanda, along Yasukuni Dori generally between Ogawamachi and Jimocho subway stations. Agree that small should be right for you. The N2Ns are pretty true to size - I'm about 31 and a M is right for me. With a 28 waist an M would be far too big.

I suspect if you are about a 30" waist then large jammers would be too big for you. I am about 31 and size M are right for me. Ha, I have a pair too and would describe them pretty much the same way! The lack of rubber in the waistband makes them a bit loose but they are not quite as saggy as the loose range.

The first seller is in Bangkok, and the suits look like asian designs. The second seller is in Korea - ditto. You would definitely need a much larger size than normal; those are asian sizings. Asian Speedo/TYR suits would have a very different fit to Western ones so don't take guidance from Western ...
